Founded in 1984, PATH (People Assisting the Homeless) provides comprehensive services across California, from homelessness prevention to permanent supportive housing.

About Our Supportive Housing Services

Our Supportive Housing Services (SHS) programs provide Intensive Case Management Services (ICMS) in a Permanent Supportive Housing (PSH) setting to individuals who have experienced homelessness and have a history of chronic health, mental health, and/or co‑occurring conditions. Services are centralized and provided on‑site to ensure ease of access to resources for long‑term housing stabilization and improved quality of life.

About the Case Manager

The Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. This position supports clients through all phases of homelessness, including on the street, transitional housing, interim housing, and permanent housing.

Position Responsibilities
  • Conduct intake and individualized needs assessment for all clients, collaborating to develop an Individualized Service Plan (ISP). This plan addresses housing goals, income maintenance/increase, and other personal goals identified by the client, including medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
  • Complete program‑specific assessments (e.g., VI‑SPDAT).
  • Evaluate strengths and challenges to address short‑term and long‑term goals; conduct a 90‑day review with the client after initial intake.
  • Maintain documentation standards required by the Program and PATH policies, including timely case file notes and a complete, accurate, current client file with all required documents.
  • Maintain confidentiality of client files per HIPAA and applicable guidelines.

  • Critical thinking and problem solving: analyze and evaluate information and situations, identify key issues, consider alternatives, and make logical decisions based on sound reasoning.
  • Ethics and boundaries: maintain ethical and professional standards, demonstrating honesty, integrity, and a commitment to doing the right thing while maintaining professional boundaries.
Preferred Qualifications
  • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
Minimum Qualifications
  • All levels of education and experience are welcome.
Mandatory Requirements
  • Employment eligibility verification.
  • CPR/First Aid training or willingness to obtain it.
  • Successful completion of Tuberculosis Test, Background Screening, and Drug Test as a condition of hire.
