Salary$23.24 to $28.28 per hour
Job SummaryPATH is seeking a Case Manager to support clients in breaking the cycle of homelessness through a “whatever it takes” approach. The position supports clients in all phases of homelessness, including street, interim housing, transitional housing, and permanent housing at our Senior Villas site.
Responsibilities- Conduct intake and individualized needs assessment and develop Individualized Service Plan (ISP) addressing housing goals, income, and other personal goals.
- Provide referrals, linkages, information, and support to resources that help clients achieve ISP goals.
- Complete program-specific assessments (e.g., VI-SPDAT).
- Evaluate strengths and challenges and conduct a 90-day review with the client after initial intake.
- Maintain documentation standards and complete case file notes timely as required by PATH policies.
- Maintain confidentiality of client files per HIPAA and applicable guidelines.
- All levels of education and experience are welcome.
- Have employment eligibility verification.
- Have or be able and willing to obtain CPR/First Aid training.
- Successfully complete Tuberculosis Test, Background Screening, and Drug Test.
- Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

- Collaboration and Teamwork – build positive working relationships with colleagues and stakeholders.
- Communication – convey information clearly in writing and verbally.
- Creativity and Initiative – take action proactively and generate innovative solutions.
- Critical Thinking and Problem Solving – analyze situations and make logical decisions.
- Ethics and Boundaries – maintain ethical and professional standards.
- Have reliable transportation, a valid driver’s license, proof of insurance and vehicle ownership, and the ability to qualify for PATH’s insurance coverage.
