Position OverviewAspire Senior Living Advance is seeking a compassionate, organized, and resident-focused Social Services Director to join our leadership team.
The Social Services Director is responsible for supporting the social, emotional, and psychosocial well‑being of our residents while serving as an advocate and resource for residents and their families. This position works closely with nursing, therapy, administration, and other members of the interdisciplinary team to ensure residents receive individualized, person-centered support throughout their stay.
The ideal candidate will have strong communication and organizational skills, knowledge of long-term care regulations, and experience working with residents and families in a skilled nursing, long-term care, senior living, or healthcare environment.
Key ResponsibilitiesConduct comprehensive psychosocial assessments to identify residents' needs, preferences, strengths, concerns, and support systems.
Develop, implement, and update individualized care plans addressing residents' social, emotional, behavioral, and psychosocial needs.
Participate as an active member of the interdisciplinary care team.
Provide residents and families with emotional support, advocacy, education, and assistance throughout the resident's stay.
Promote and protect resident rights, dignity, privacy, choice, and independence.
Assist residents and families with adjustment to long-term care and changes in health, function, or living arrangements.
Coordinate discharge planning and transitions of care, including referrals for home health, community services, equipment, transportation, and other post-discharge resources.
Connect residents and families with appropriate community resources, support programs, financial resources, and outside services.
Assist with concerns, grievances, conflicts, and challenging situations while promoting effective communication and positive resolution.
Provide crisis intervention and utilize appropriate de-escalation techniques when needed.
Maintain accurate, complete, and timely documentation of assessments, care plans, interventions, progress notes, and discharge planning activities.
Educate staff regarding resident rights, psychosocial needs, communication, cultural sensitivity, and other social-service-related topics.
Participate in care conferences, quality assurance initiatives, resident/family meetings, and other facility programs as needed.
Ensure compliance with applicable federal, state, and local regulations governing social services and resident care in long-term care.
Collaborate with facility leadership to identify opportunities to improve resident satisfaction, quality of life, and social services programming.
Perform other duties as assigned.
Bachelor's or master's degree in Social Work or a related field, as required based on facility size and applicable regulations.
State licensure or certification when required by applicable regulations.
Previous experience in long-term care, skilled nursing, senior living, healthcare, social services, case management, or a related setting preferred.
Knowledge of resident rights, psychosocial assessments, care planning, discharge planning, and long-term care regulations.
Strong interpersonal, communication, problem-solving, and organizational skills.
