• Perform initial nursing assessments and ongoing cognition screens at start of care and develop an individualized care plan for each new client including recommendations for care hours.
• Perform follow‑up visits as per state licensing requirements and at client status changes.
• Accurately monitor and record data regarding patients' conditions; document all provided care services.
• Collaborate with community care partners to support the provision of quality care to each client.
• Supervise, support and train CNAs to ensure they are fully equipped to provide the highest quality care to our clients.

• Must be a Registered Nurse licensed in Maryland.
• Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations.
• At least 1 year of experience as an RN.
• Experience working with seniors in home care preferred.
• Experience in care coordination between physician, clients, referral sources.
• Excellent communication skills.
• Great problem‑solving skills.
• Basic IT skills.
