Minimum Requirements ? Board Certified in Family Medicine ?

Certifications:

ACLS and BLS ? Active and unrestricted medical license in Alabama (by start date) ? Minimum 3 years of outpatient primary care experience ? Proficiency in chronic disease management (e.g., diabetes, COPD, CHF, obesity) ?

Experience with electronic medical records ?
