• Implement care plans for clients developed by the supervisory Physical Therapist
• Provide direct physical therapy treatment to clients as assigned in the center and/or participant homes
• Instruct clients and family members in the use and care of therapeutic appliances and devices
• Assist in assessing the client’s need for physical therapy devices and interventions
• Report to physician, Physical Therapist, Registered Nurse, or management regarding client responses to treatment or changes in condition

• Graduation from a two-year college-level program approved by the American Physical Therapy Association
• Minimum of six months experience under a qualified Physical Therapist; home health experience preferred
• Ability to function in a practice environment with minimal direct supervision
• Responsibility for maintaining clinical practice skills and adhering to agency policies and procedures
• Current Driver’s license and reliable transportation

• Experience in home health settings
• Strong communication skills with clients and families
• Ability to work collaboratively in a team environment
• Commitment to ongoing professional development
