Position: RN Case Manager (Exempt)
Overview

Qualifications

Minimum Qualifications:

* Associate Degree in Nursing

* Working knowledge of acute care, rehabilitation, or home care

Licensure/Certification /Registration:

* One of the following:

* Current Alabama RN license

* Current Multi State RN License in accordance with Nurse Licensure Compact (NLC) for Alabama
* * Infirmary Health abides by the NLC requirements and guidelines for the state of Alabama

For specific areas CRRN (Certified Rehabilitation RN) is required (or eligibility)

Desired

Qualifications:

* BSN preferred;
Master's degree in a clinical specialty desirable

* Some background in management, clinical education or case management

* National certification applicable to area of clinical practice

Responsibilities

Utilizes a multi-disciplinary plan of care to manage a defined population of patients/clients providing coordination of patient care which meets IHS's standards of quality, efficiency and desired outcomes.
