Licensed Clinical Social Worker; LCSW | Geriatrics

We’re working on behalf of a healthcare organization whose PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to organizational values, team culture, and mission.

We seek a California licensed LCSW with five (5) or more years of experience for the role of Behavioral Health Specialist. The person in this role will works alongside the Social Work team and report to the Social Work Manager. Following the PACE (Program of All-Inclusive Care of the Elderly) care model, she/he will serve on a clinical interdisciplinary team to represent & advocate for each participant’s behavioral health needs.

She/he will conduct behavioral health assessments to determine the behavioral health care needs, preferences and goals of the participants and actively participate in Interdisciplinary team meetings to develop discipline-specific goals and interventions. She/he will deliver and document behavioral health interventions as agreed upon in the participants’ care plans including but not limited to symptom management, counseling, crisis intervention, arranging necessary resources and services, assisting with care transitions, providing individual and group counseling and care management.

She/he will facilitate individual and group therapies in various forms as appropriate such as art and music therapy, psychosocial drama, counseling, and discussion that restore or expand participants’ social roles.

Candidates must have a Master’s Degree in Behavioral Health (MSW) and be licensed by CA Board of Behavioral Sciences as an LCSW. Must have a minimum of five (5) years of behavioral health experience conducting therapy, completing behavioral health assessments, and managing care planning needs. Must have a minimum of two (2) years of experience with a frail or elderly population.

The role offers highly competitive compensation (salary to $119,171 based on experience) and Medical insurance coverage (Medical, Dental, Vision) as well as 17 days of personal time off (PTO), 12 holidays observed annually, and sick time. 401 K savings + match also offered.
