Certification Details
- The Registered Dietitian is responsible for developing and implementing the Welbe Health nutrition services program working in close collaboration with the Interdisciplinary Team (IDT).
- Deliver nutrition services such as assessments and counseling to participants.
- Function as a liaison between Welbe Health and food and supplement service providers.
- Develop menus and ensure compliance with regulations pertaining to kitchen and meal service delivery.
- Collaborate effectively with colleagues and stakeholders to promote Welbe Health values, team culture, and mission.
- Conduct nutrition assessments to determine nutrition needs, preferences, and goals of participants.
- Participate in IDT meetings to develop participant care plans.
- Deliver and document direct care nutrition interventions according to physician orders or care plans, including implementing and monitoring special diets and meal intake, and providing nutrition counseling to participants and families.
- Oversee menu planning and meal delivery program to ensure safety and regulatory compliance and correct special diets.
- Oversee coordination of supplements as ordered.
- Provide staff training to day center staff on meal delivery program as needed.
- Work with PACE Physician and IDT members to support smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
- Support intake and enrollment process to ensure timely and effective participant enrollment.
- Remain alert to input from team members, participants, and caregivers and update IDT promptly on changes in participants’ condition or psychosocial status.
- Be legally authorized to perform in position location and act within scope of authority.
- Willingness and ability to work varied schedule including evenings, nights, and weekends.
- Complete all required documentation timely and accurately.
- Protect privacy and maintain confidentiality of company procedures and information about team members, participants, and families.
- Follow Welbe Health policies and procedures and participate in required Quality Improvement activities, staff training, and meetings.
- Communicate regularly with supervisor and team regarding workload and priorities.
- Timely completion of mandated trainings, education, and occupational health screenings.
- Exercise flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Job Details
- Minimum of four years’ experience managing nutrition services in a similar setting.
- Minimum of one year of experience with a frail or elderly population.
- Demonstrated ability to mentor and develop team members.
- Excellent organizational and communication skills.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
- Commitment to unlocking the full potential of vulnerable seniors.
- Registered Dietitian (RD) Credential by the Commission on Dietetic Registration (CDR).
- BLS certification.
Job Requirements
