Position SummaryReporting to the Stroke Program Coordinator, the Stroke Program Nurse Navigator/Physician Assistant provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members and support services. The role coordinates multidisciplinary care based on evidence‑based practice and is responsible for providing stroke education, delivery and oversight of patient care, including post‑discharge follow‑up. The position works in collaboration with Stroke Program key stakeholders to meet certification requirements, review data, and lead performance improvement projects while incorporating the patient experience in all aspects of care.
This is a full‑time exempt position.
- CA RN/NP or CA Physician Assistant license
- BLS, ACLS, NPI
- NIHSS and ASLS within 3 months of hire/transfer; ENLS within 6 months of hire/transfer
- SCRN/CNRN/NVRN preferred
- Strong ICU, neuro, or ED experience preferred
