Assistant Director of Patient Care Services

Healthy Living at Home - Modesto, LLC is in search for an experienced and professional Registered Nurse to be our Assistant Director of Patient Care Services ADPCS to assist in the implementation and overall clinical direction of our home health clinical services in the greater Modesto, CA operational area.

The Assistant Director of Patient Care Services is the Registered Nurse (RN) responsible to assist in the overall direction of clinical services, establishing, implementing and evaluating goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organizational philosophy.

• Receives case referrals. Reviews available patient information related to case, including home visits, to determine home health care needs. Assigns appropriate personnel to case as needed. Conferences with Medical Director and/or Director of Patient Care Services regarding any questions about an individual’s eligibility for services.
• Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home health services; and is available at all times during operating hours to assist clinicians as appropriate.
• Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
• Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
• Attends case conference meetings with home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
• Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and home health personnel.
• Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new

• Assists Clinical Director/Director of Patient Care Services and other supervisory home health personnel in the planning, implementation and evaluation of inservice and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.
• Complies with accepted professional standards and principles.
• Assess health needs of community and assists in the development of plan(s)/ program(s) to meet needs.
• Participates in public relation and community activities that promote the organization’s role as an effective member of the home health and healthcare delivery system. Promotes customer service orientation to all home health personnel.
• Participates actively in quality assessment performance improvement program.
• Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.

• Current unencumbered Registered Nurse (RN) license in location state
• A graduate in Nursing from an accredited program by the National League for Nursing, AND
• Three or more (3+) years of Registered Nursing experience
• Two or more (2+) of the last five (5) years in Registered Nursing Home Health organization
• One or more (1+) years of supervisory/management experience in home health care organization
• Current CPR card
• Demonstrated ability to supervise and direct professional administrative personnel
• Knowledge and expertise in clinical decision-making and Quality Management review and Evaluation
• Ability to market and deal tactfully with customers and the community
