Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (40 hrs per week) provider for our Allure of the Quad Cities Center located in Moline, IL. This position can be filled by a nurse practitioner or physician assistant. The salary range for this position is $105,000 - $120,000 per year.

• The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
• Collaborates on‑site with the Center Leadership Team for center specific goals and priorities.
• Collaborates with the Medical Director and/or Attending Physician for patient specific medical care.
• Provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
• Works collaboratively with Center Leadership to achieve Align Med Partners mission of improving the lives we touch through delivery of high quality health care and everyday compassion.

• Must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified Nurse Practitioner with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations may be considered based on experience and business need.
• Physician Assistant must be a graduate from an ARC‑PA accredited program, have passed the PA National Certifying Exam (PANCE), hold state licensure and NCCPA certification.
• Minimum of two years experience working with adult or geriatric population is preferred.
• APRN with an unrestricted professional license and DEA for each state in which they practice.
• PA with an unrestricted professional license and DEA for each state in which they practice.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service. Eligibility for full‑time benefits requires a minimum of 30 hours per week. Certain benefits, including Health plans, are not available to part‑time employees working between 16 and 29 hours per week.

Posted Salary Range: USD $105,000 – $120,000 per year.
