Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends). We are seeking a full‑time (40 hours per week) provider for our Allure of Moline Center located in Moline, IL. This position can be filled by a nurse practitioner or physician assistant. The hourly range for this position is $55–$58 per hour.

Our physicians, nurse practitioners (NPs) and physician assistants (PAs) are supported by a clinical leadership team with a decades‑long track record of excellence in post‑acute and long‑term care.

• Reports directly to Director of Clinical Practice and/or Area Clinical Practice Manager; collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities.
• Collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.
• Provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
• Works collaboratively with Center Leadership to achieve Align Med Partners mission of improving the lives we touch through delivery of high quality health care and everyday compassion.

• Licensed registered nurse (RN) or nurse practitioner (NP) from an accredited program; must hold an NP degree with a concentration in Older Adult and/or Family NP. Other concentrations are considered based on experience and business need.
• Alternatively, Physician Assistant graduate from an ARC‑PA accredited program, passed PA National Certifying Exam (PANCE), with state licensure required and NCCPA certification required.
• Minimum two years experience working with adult or geriatric populations preferred.
• In states where practice allows, APRN with an unrestricted professional license and DEA registration; PA with unrestricted license and DEA registration.
