Care Manager Registered Nurse - Field; Western Illinois-Quad

The Care Manager Registered Nurse - Field position at CVS Health involves collaborating with members, providers, and community organizations to address the healthcare and social needs of dual eligible populations. • Pay: $66,575 - $142,576/year
• Location: Moline, IL
• Schedule: Full-time
• Travel: Up to 75% travel required

• Utilize critical thinking and judgment to inform the case management process.
• Facilitate appropriate healthcare outcomes for members by assisting with appointment scheduling.
• Identify and assist members with accessing benefits and education through care management tools.
• Collaborate with teams virtually using various tools.
• Demonstrate proficiency with standard corporate software applications.
• Maintain effective communication with members and providers.
• Work independently and manage multiple systems and applications.
• Resolve common technical issues independently.

• Associate's degree required, bachelor's preferred

* Experience:

• Minimum 3-5 years clinical practical experience
• 2-3 years in Care Management, discharge planning, and/or home health care coordination preferred
