Shape the future of care navigation as a Care Concierge with Carewell in Nova Scotia, Canada. Engage with members, facilitate enrollment, and provide essential support to ensure their health needs are met.
This role focuses on supporting a pioneering care navigation program a Care Concierge, your daily tasks will include enrolling members, conducting welcome visits, and coordinating care navigation to address members' needs effectively. This position demands a strong understanding of healthcare processes and relationship-building with older adults while flexibly adapting to real-time demands.
Key Responsibilities:
• Conduct outreach to prospective members and gain trust
• Perform welcome visits for newly enrolled members
• Coordinate with healthcare providers and community resources
• Document member interactions consistently and accurately
• Support ongoing care navigation by closing care gaps
Requirements:
• 1+ years of member-facing healthcare experience
• Strong communication skills with older adults
• Familiarity with EHR systems and care coordination
• Ability to work Monday–Friday, 9 am–6 pm EST
• Metrics-driven and comfortable with ambiguity
