Hourly Rate of Pay: $48.00 - $51.00 Carecor is looking for dedicated and compassionate Registered Nurse (RN) - Community who will provide comprehensive, patient-centered nursing care to individuals and families in community and home care settings. The RN works independently and collaboratively with interdisciplinary healthcare teams to assess, plan, implement, and evaluate care that supports clients in maintaining their health, independence, and quality of life.
This role involves managing a diverse caseload of patients with acute, chronic, complex, and palliative care needs while promoting health education, disease prevention, and wellness. The Community RN delivers care in accordance with professional nursing standards, organizational policies, and applicable regulatory requirements.

Key Responsibilities
Provide comprehensive nursing care to patients in community and home care settings.
Conduct patient assessments and develop, implement, and evaluate individualized care plans.
Administer medications, treatments, and wound care as prescribed.
Monitor patient health status and communicate changes to physicians and members of the healthcare team.
Educate patients, families, and caregivers on health management, treatment plans, and self-care techniques.
Promote patient independence, safety, and well-being while supporting care in the home environment.
Maintain accurate and timely clinical documentation in accordance with professional and organizational standards.
Coordinate care and collaborate with interdisciplinary teams to ensure seamless service delivery.
Travel between patient homes and community locations as required to provide care.
Adhere to infection prevention and control practices, workplace safety standards, and nursing best practices.
