Sentrex Health Solutions seeks a Field Nurse Case Manager for Patient Programs. You will coordinate patient support services, assist healthcare providers, and help patients access drug therapy. The position requires English-French bilingualism, clinical experience, and extensive regional travel.
Key Responsibilities:- Register healthcare providers and patients according to program protocols
- Provide disease, therapy, adherence, and case-management support
- Monitor patients, collect data, and report adverse events
- Coordinate funding, coverage renewals, and reimbursement support
- Liaise with physicians, pharmacies, and infusion service providers
- Registered Nurse in good standing with the provincial nursing college
- Two to four years of clinical experience
- English-French bilingualism is required
- Oncology, gastroenterology, or rheumatology experience
- Ability to resolve conflict professionally and work within teams
Support Sentrex patient programs through bilingual nursing care, clinical monitoring, provider coordination, and regional clinic visits.
