Care Team Nurse (LPN/RN)

Job Status:
Full‑time;
Monday – Friday, 8 am – 5 pm (optional 4/10 work schedule)
- Provide practice management support to the clinical team, embracing the Patient Centered Primary Care Home (PCPCH) by recognizing the patient as the central focus.
- Collaborate with other health care professionals to address patients’ wellness, acute illness, chronic disease, disability, and end‑of‑life needs.
- Offer telephone advice to patients with minor acute problems; conduct patient education on self‑care; support ongoing management of stable chronic disease; monitor acutely ill patients during routine office visits; perform procedures within scope of nursing practice.
- Oversee provider‑assigned patient panels, directing and facilitating care and employing the nursing process: assessment, planning, intervention, and evaluation.
- Participate in ongoing management of patients with stable chronic diseases, communicating with patients in person and by telephone regarding management, coordination of treatment plans and services, and follow‑up when indicated.
- Manage triage, intake and monitoring of patients with acute medical conditions; provide nursing intervention when appropriate in absence of PCP.
- Under direction of PCP, provide patient education on self‑care (medications, nebulizers, inhalers, diabetes management, wound care, self‑injection).
- Assist providers with patient examinations, procedures (minor surgery, casting, nebulizer treatments, spirometry, EKG) and medication administration as needed.
- Observe, analyze and interpret clinical data; recognize abnormal or critical findings and notify provider as appropriate.
- Current Oregon RN or LPN license.
- BLS certification consistent with AHA Guidelines for CPR/ECC covering breathing and cardiac emergencies.
- Clinic experience preferred.
- Independent judgment and decision‑making.
- Team‑work skills; ability to prioritize competing responsibilities under pressure.
- Problem‑solving skills.
- Personal flexibility to perform in an ever‑changing work environment with frequent interruptions.
- Ability to manage high volumes of patients in a short span while dealing with unpredictable issues.
