About the Role

Part‑time Hospice Registered Nurse Case Manager located in Gwinnett County and Monroe, GA region and surrounding areas. As an interdisciplinary team member, the RN Case Manager works under the Director of Nursing, attending physician and the interdisciplinary group to provide skilled nursing assessments, planning and care to maximize the comfort and health of patients and their loved ones/caregivers.

Responsibilities
  • Manage an assigned patient caseload by the Patient Care Coordinator or designee.
  • Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process.
  • Initiate communication with attending physicians, hospice staff members and other agencies as needed to coordinate optimal care.
  • Maintain regular communication with the attending physician concerning patient and loved‑one needs.
  • Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in and contribute appropriate information to interdisciplinary group meetings.
  • Coordinate physical care of the patient by teaching primary caregivers, volunteers, or providing direct care as appropriate.
  • Meet regularly with hospice nursing staff to review problems or unique issues from caseloads and share professional support.
  • Attend patient care conferences and facilitate discussion of issues from caseloads for full staff discussion, consultation and evaluation.
  • Supervise and document the nursing assessment (NAC) every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
  • Assume the role of associate nurse during off‑hour times or as requested to fill patient/family needs during the absence of other primary nurses.
  • Provide appropriate support at time of death and period of bereavement.
  • Participate in hospice orientation and in‑service training programs for professional staff.
  • Participate in agency and community programs as required to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and rules and regulations of state and federal bodies governing hospice care.
  • Accept other assignments as appropriate.
Qualifications
  • Current license to practice as a registered nurse in the State of GA.
  • At least 1–2 years of professional nursing experience, preferably including hospice care.
  • Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
  • Skilled in nursing practice, able to cope with emotional stress and tolerant of individual lifestyles.
  • Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports.
  • Team player who supports an interdisciplinary team concept of caring for patients and loved ones/caregivers.
  • Valid driver’s license, a good driving record, a reliable vehicle, and the ability to drive year‑round to locations.
  • Ability to take direction, work independently, and effectively as part of a multidisciplinary team in a positive, professional manner.
Compensation

Starting at $51,000 per year.
