Staff Pharmacist

Job Description
The pharmacist dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration with other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use, oversees the work of the support staff, and documents any relevant interventions. The position requires state certifications, familiarity with standards and practices, and relies on experience and judgement to plan and accomplish goals.
Responsibilities- Reviews and interprets patient prescriptions and profiles to prevent overdoses, incompatibilities, and toxicity; ensures high quality pharmacy services and patient care.
- Accurately processes orders and IV admixtures.
- Demonstrates understanding of formulary system and ensures compliance.
- Documents all clinical interventions in the department‑approved tracking system.
- Supervises support personnel and reviews all functions performed by them.
- Removes out‑of‑date medication from inventory and checks patients accordingly.
- Maintains physical inventory to ensure adequate supply of medications.
- Completes all assigned daily tasks and assists others when time permits.
- Available to fill in for staff pharmacists as needed.
- Performs clinical calculations for pharmacokinetics and dosing based on patient parameters.
- Advises other health care professionals about drug properties, administration, stability, and compatibility.
- Uses references and computer programs to accurately dose and understand pharmacokinetics.
- Provides in‑service education to health care professionals and participates in community education activities.
- Records and reports data, including interventions, adverse drug reactions, medication errors, Drug Utilization Evaluations, and cost‑effective therapeutic interchanges.
- Assesses situations and chooses appropriate actions; communicates effectively with diverse individuals.
- Communicates with physicians and nursing staff about drug availability, side effects, and incompatibilities.
- Manages support staff and provides direction in daily responsibilities.
- Expeditiously resolves communication deficits.
- Retail experience and working knowledge of 340B and Patient Assistance programs.
- Bachelor's Degree.
- Licensed as a Registered Pharmacist (state appropriate); BLS (Basic Life Support / CPR).
