Oncology Care Navigator; RN Seamless Patient Transitions

Ochsner Health System is seeking a dedicated RN to act as a liaison across patient, caregiver, and multidisciplinary teams to ensure seamless transitions and optimal care outcomes. This role supports discharge planning and post-acute care coordination to improve patient satisfaction and reduce readmissions.

The successful candidate will have an active RN license, BLS, and 2+ years of nursing experience, with strong communication skills and familiarity with electronic health records.
