Occupational Therapist – Home Health, Senior Living

BAYADA Home Health Care is currently seeking an experienced Occupational Therapist for a full‑time opportunity performing home health visits with adult and geriatric patients on a per‑visit basis in senior living communities throughout Middlesex/Somerset County.

As a home‑care Occupational Therapist, you will be an integral member of a multi‑disciplinary health‑care team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Sign‑on bonus: $10,000 for full‑time OT's.

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities.
• Professionally operate at the top of licensure utilizing current research‑based approaches to care to maximize functional independence for clients.
• Work closely with home‑health therapy partners to appropriately transition care from home‑health to outpatient services, achieving client‑centered goals in their living environment.
• Educate the client, family, and community caregivers on the care plans created to maximize functional independence.
• Perform evaluations, treatments, and proper discharge planning using the outpatient EMR platform to document individualized treatment approaches.

• A current NJ Occupational Therapist license.
• Prior home‑care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior EMR experience preferred.

Base Pay: $60.00 – $68.00 per point, depending on qualifications and subject to negotiation.
