• Provide exceptional care in nursing homes across Monroe and Freeport areas
• Lead a dynamic team of 3 specialized Geriatric Nurse Practitioners

• Practice with high autonomy and independence

• Board certification in Family Medicine or Internal Medicine
• Dual Licensing, WI and IL (or ability to obtain)
• Post-acute and long-term care experience preferred
• Passion for compassionate, patient-centered elder care
