SUMMARY

The Case Manager – CYF (CST) is responsible for the coordination of Green County’s Coordinated Services Team (CST) program under the supervision of the Children, Youth, and Families Supervisor. Responsibilities include developing the Agency’s CST Plan and Program Outcomes, recruiting and training CST facilitators, completing assessments and determining program eligibility, facilitating Coordinated Services Teams, and providing other services within the Children, Youth and Families Unit as needed.

This position works in conjunction with Case Workers and staff to coordinate services as necessary and to report child abuse/neglect concerns.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Meet with families to develop an understanding of their story and program eligibility
  • Assist the family in identifying the initial team members
  • Complete Strengths and Needs Assessment (CANS) with the family and address immediate needs
  • Maintain CANS certification
  • Prepare team member for participation on a CST
  • Facilitate team meetings
  • Develop and maintain a strength‑based agenda
  • Help team develop ground rules
  • Facilitate the group process and build agreements on needs, strengths and action plan
  • Assure family and other team member participation
  • Manage conflict between team members
  • Model a non‑judgmental, family‑centered approach
  • Document the Plan of Care and guide team process to implement and manage the Plan of Care over time
  • Integrate the results of the team meeting into Plan of Care
  • Document Crisis Response Plans and distribute as necessary; ensure evaluation of plan
  • Work with the family and team reviewing and modifying the Plan of Care
  • Maintain family and team ownership of Plan of Care over time
  • Deliver direct services only when necessary
  • Begin development of phase‑out/transition plans to discuss with team
  • Keep appropriate documentation of contacts in PPS and eWISACWIS
  • Determine eligibility for targeted case management or CCS
  • Maintain training requirements for CCS facilitator
  • Maintain the CST Coordinating Committee according to CST requirements
  • Accept referrals from community organizations and schools and participate in community awareness activities
  • Coordinate with Family Ties and Parent Support Specialist
  • Develop and coordinate Parent Support Groups
  • Maintain records and reporting requirements for CST grants and complete regular required reporting and continuation plans
  • Prepare for audits and site visits as required for CST funding
  • Attend regular trainings and CST related state wide/regional meetings
  • Meet with ongoing staff who are facilitating teams and assist them with completing CST requirements, reporting and targeted case management if necessary
  • Participate in creating a crisis plan with the family when necessary
  • Maintain skill and competency levels as policies and regulations change by attending and participating in staff meetings, supervision times and other regularly scheduled meetings; develop professional knowledge and skills by attending training events, conferences and workshops.
  • Complete DCF 43 training requirements including pre‑service and foundation training. Regularly attend Safety Training to maintain current on safety standards
  • Explain the scope of the agency's services and discusses the client's rights and responsibilities in relation to the use of such services
  • Participate in committees and task forces as assigned as a liaison to various community agencies and organizations
  • Maintain confidentiality of family‑related information and maintain respectful treatment of the family
  • Transport children and juveniles in personal vehicle as needed
  • Work with other departmental units to assist the family (Mental Health, AODA, CCS, Economic Support, CLTS, Birth‑3, Public Health, WIC, Child Support, etc.)
  • Establish and maintain effective working relationships with coworkers, the public, community resources, county and governmental officials, and others
