POSITION STATUS:
Full Time

Responsibilities
  • Care Management Site Supervisor is responsible for the supervision of care management staff and the daily care management operations of the program. This includes supervision of Health Home staff, ensuring quality of care management services provided to clients, management of staff documentation requirements, and staff training.
  • The site supervisor will ensure provision of adequate clinical support to clinically complex clients and clients in crisis.
  • Provide direct supervision to care managers to assist with the development of effective interventions and Care plans
  • Ensure staff is kept informed of Health Home policies and procedures as well as departmental priorities. Receive appropriate trainings to maintain clinical standards
  • Monitor Health Home compliance and quality assurance
  • Provide crisis intervention to clients in psychological distress
  • Complete enrollment and service data tracking reports monthly or as needed
  • Continually review the service delivery process to exceed customer expectations
  • Review and approve client documentation as assigned
  • Participate in quality assurance and quality improvement activities
  • Conduct staff meetings, trainings and supervision
  • Occasional field work may be required as needed to achieve client outcomes and health home requirements
  • Performs other duties as requested
  • Facilitation of bi weekly Clinical Case Review with the Care Managers

Qualifications
  • Master’s Degree in Social Work, Psychology or Human Services preferred
  • One year post graduate experience working with individuals or families who have a history of mental illness, substance use, homelessness or HIV/AIDS
  • Previous supervisory experience required
  • Strong leadership ability
  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Good problem-solving, decision-making and judgment skills
  • Ability to work in a team management system
