RN case Manager

Contract Duration: 12 months with possible extension

Schedule: Monday – Friday, 8 am – 4:30 pm | Full-time, 40 hours per week

Setting: Hospital and Outpatient Clinic

Job Description: Seeking an experienced RN Case Manager to provide patient-centered care coordination. The RN will be responsible for assessing patient needs, developing care plans, and ensuring continuity of care across multiple disciplines and settings.

Requirements:

Active, unrestricted Registered Nurse (RN) license in any U.S. state or territory.

Minimum 2 years of case management or care coordination experience in hospital or outpatient settings is preferred.

Current Basic Life Support (BLS) certification.

Knowledge of care coordination models, discharge planning, and utilization review.
