We are seeking a skilled

Adult Nurse Practitioner to provide comprehensive primary care in ambulatoryand community-based settings in Montebello, California. This role offers the opportunity to deliver patient-centered care while supporting preventive health, chronic disease management, and long-term wellness.

• Provide comprehensive primary care services to adult patients.
• Evaluate, diagnose, andmanage acute and chronic medical conditions.
• Perform preventive care,health screenings, and routine assessments.
• Manage medications and monitor treatment effectiveness.
• Develop individualized careplans and provide patient education.
• Support long-term disease management and promote healthy behaviors.
• Collaborate with physicians and other members of the healthcare team.
• Maintain accurate and timely clinical documentation.

• Master’s or Doctoral degree from an accredited Nurse Practitioner program.
• Current, unrestricted
  
  California Nurse Practitioner license.
• National Board Certification as an Adult Nurse Practitioner or appropriate adult-focused primary ca recertification.
• Current DEA registration or eligibility to obtain one.
• Experience providing primary care in an ambulatory or community-based setting preferred.
• Strong clinical assessment,diagnostic, communication, and patient education skills.
• Commitment to evidence-based, patient-centered care.
