As a Primary Care Nurse, your core responsibilities will include patient assessments, chronic disease education, and collaborating with a multi-disciplinary team. You will manage a diverse caseload through both scheduled and walk-in appointments, ensuring quality care for patients' physical and mental health.

Key Responsibilities:

• Provide primary health care services to patients

• Conduct screenings and assessments for diverse needs

• Implement chronic disease management and education programs

• Collaborate with healthcare professionals on care planning

• Assist with walk-in and scheduled patient visits

Requirements:

• At least one year of relevant nursing experience

• Baccalaureate Degree in Nursing from an accredited institution

• Registered Nurse license from a provincial professional college

• BLS Certification for Healthcare Provider mandatory

• Current N95 fit certificate and WHMIS Certification required
