Overview

The Care Coordinator is responsible for completing psychosocial assessments and participating in ongoing treatment planning for psychiatric patients of all ages, including children, adolescents, and adults, across the care continuum. The role involves assessing and addressing patient support systems and care management needs related to complex psychiatric and psychosocial problems. The Care Coordinator is a member of the multidisciplinary treatment team responsible for planning and implementing care interventions, including group and individual treatment modalities.

The position provides emotional and practical support to patients and their support people to enhance functioning and meet individualized treatment goals.

Responsibilities
  • Complete psychosocial assessments and participate in ongoing treatment planning for psychiatric patients of all ages.
  • Assess and address patient support systems and care management needs related to complex psychiatric and psychosocial issues.
  • Plan and implement care interventions, including group and individual treatment modalities, as part of a multidisciplinary team.
  • Provide emotional and practical support to patients and their support persons to enhance functioning and achieve treatment goals.
  • Support care across the care continuum, including psychiatric consult and observation, and adolescent residential programs.
  • Help patients and their families understand and manage behavioral health factors, including legal, educational, financial, occupational, transportation, and insurance issues.
  • Facilitate referrals to services and programs when indicated; learn about effective local and regional resources and build collaborative relationships with providers.
  • Provide psychoeducation individually and in groups; attend treatment meetings and work closely with therapists, psychiatrists, and other behavioral health staff.
  • Participate in interventions to support milieu management and proactive de-escalation.
  • Report to the Assistant Director of Acute Psychiatric Services; receive clinical and quality feedback from Behavioral Health leadership.
  • Demonstrate commitment to continuous learning, innovation, quality of care, and teamwork in a multidisciplinary environment.
Qualifications
  • Experience working with psychiatric patients.
  • Experience leading individual and group treatment modalities.
  • Knowledge of local and regional psychiatric services, legal resources, community services, and public insurance (Medi Cal) programs; experience coordinating discharge planning and continuity of care.
Education
  • Master’s degree in Clinical Social Work, Counseling, Clinical Psychology, or Master’s Degree in Marriage and Family Therapy.
Licensure/Certification s
  • Licensure with the California Board of Behavioral Sciences as an LCSW, LMFT, or LPCC is required.
  • American Heart Association Healthcare Provider BLS certification required.
  • Organization-approved de-escalation training; 5150 training as required by Monterey County to obtain 5150 certification;
    Live Scan fingerprint clearance required by the Department of Healthcare Services.
  • Certification in an evidence-based practice is preferred (e.g., CBT, DBT, ACT, EMDR).

  • Pay Range (based on experience): $49.77 to $66.56 per hour
  • The hours worked determine when a shift differential is paid:
    Evening differential $3.39;
    Night differential $5.09.
