Occupational Therapist Role SummaryAs an Occupational Therapist, you'll evaluate clients' functional abilities and daily activity demands, identify areas of deficit, and deliver evidence-based interventions that enhance quality of life and support independence. You will implement the standards of care for occupational therapy in accordance with the Service Agreement, the physician's orders, and the established plan of care.
Key Occupational Therapist Responsibilities- Deliver occupational therapy services per physician orders and the client's plan of care.
- Assess and document therapeutic, rehabilitative, and functional status.
- Develop and update an individualized plan of care that aligns with assessment findings.
- Collaborate in creating and refining the client's overall treatment plan.
Educate clients and family/caregivers on the selection, use, and maintenance of adaptive equipment and assistive devices. Monitor and report client progress, treatment response, and any change in condition to the Director of Nursing and the physician. Participate in interdisciplinary case conferences to coordinate care. Identify needs for additional services and communicate recommendations to the Director of Nursing. Prepare and submit timely clinical notes and progress summaries tied to goals and outcomes (submitted at least weekly to the Director of Nursing).
Contribute to discharge planning as appropriate to support safe transitions. Provide in-service education and training for agency staff.
- Graduated from an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or predecessor organizations.
- Current and unrestricted Occupational Therapist (OT) license in the State of California.
- Two (2) years of appropriate experience as an occupational therapist.
- Current CPR certification.
- Valid driver's license.
- Home Health experience.
- Reliable transportation and auto insurance; comfortable traveling within assigned territory.
- Strong assessment, documentation (EMR), and communication skills; ability to work independently in the home setting.
- $100 per hour + mileage reimbursement.
- Flexible scheduling and manageable caseloads.
- Supportive clinical leadership and responsive office team.
- Paid training/orientation; CEU support (if offered).
- Paid Sick Leave.
- Part‑time caseload with flexible day(s)/time(s).
- Service area:
Monterey County - Monterey, Carmel, Pacific Grove, Seaside, Marina, Salinas, Pebble Beach, Carmel Valley, and surrounding neighborhoods. - Mileage: reimbursed for travel between patient visits within assigned territory.
