Assignment Highlights:

• Collaborate with an interdisciplinary care team focused on meaningful rehabilitation outcomes and safe patient transitions.

What You Will Do:

• Conduct thorough occupational therapy evaluations that identify functional limitations, strengths, and individualized rehabilitation needs.
• Create and deliver treatment plans that improve activities of daily living, mobility, coordination, strength, and functional performance.
• Teach patients and caregivers adaptive techniques, home exercises, and safe use of adaptive equipment to promote lasting independence.
• Document evaluations, interventions, progress, and outcomes accurately in Net Health, also known as Rehab Optima.
• Partner with nursing, physical therapy, physicians, and other healthcare professionals to support care planning and discharge planning.

Required Qualifications:

• You hold an active, current California Occupational Therapist license and are eligible to practice independently.
• You bring at least one year of recent occupational therapy experience in a skilled nursing facility or post-acute rehabilitation setting.
• You are confident using Net Health or Rehab Optima for timely, compliant therapy documentation.
• You have strong clinical judgment and can develop individualized therapeutic interventions for patients with varied rehabilitation needs.
• You communicate clearly with patients, families, and interdisciplinary team members while maintaining a compassionate, professional approach.

Preferred Strengths:

• Experience supporting patients with complex rehabilitation needs, transitional care, or discharge planning will help you make an immediate impact.
• Knowledge of adaptive equipment, care planning, and activities of daily living interventions is highly valued.
• Current BLS certification and experience in acute care, outpatient, or inpatient rehabilitation are welcome additions.

Pay and Benefits:
