Healthy Living at Home - Monterey, LLC is in search for a professional, knowledgeable and detail oriented Home Health Intake Nurse (LVN) to provide intake services and assessment for our Home Health patients in the greater Monterey, CA metropolitan area.

Position Overview

The Intake Coordinator - Licensed Vocational Nurse (LVN) is responsible for managing all aspects of the patient intake process to include obtaining referral information, physician orders, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Essential Job Functions

• Responsible for obtaining accurate referral information
• Obtains specific physician orders for recertification of home care services
• Ensures that physician’s orders are communicated to agency clinicians and staff with accuracy, thoroughness and understanding.
• Uses office and other patient care software
• Addresses and supports cultural practices, as along as such practices do not harm others or interfere with the planned course of medical therapy
• Participates in agency’s information management and performance improvement programs
• Complies with agency’s policies
• Assists administration in meeting the JCAHO, federal, state and location regulations and standards
• Other administrative duties as assigned

Basic Qualifications

• Graduate of an accredited nursing school/program
• Current unencumbered Licensed Vocational Nurse or Licensed Practical Nurse license
• One (1) year of experience in Home Health
• Demonstrated ability to work alongside professional and administrative personnel.
• Knowledge of corporate business management, governmental regulations, Joint Commission standards, and private payer practices.
• Demonstrates good communication, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Preferred Qualifications

• Current unencumbered Registered Nurse license
• Two (2) year of experience in Home Health
• One (1) year staffing and administrative experience

Physical Requirements

Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary.

The employee must be able to lift and/or move up to 10 lbs. on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
