Overview

Ultra Health Providers is offering an opportunity for a dedicated nurse practitioner (NP) to provide high-quality primary care in a skilled nursing facility setting. The position is full-time, Monday – Friday, and involves working with a collaborative team to serve patients, clients, and colleagues.

You will provide primary, urgent and preventive care for residents in the assigned skilled nursing facility and may also provide care to assisted living facility residents. You will verify admission orders, conduct initial assessments, perform physical exams, review medications, and conduct depression screening. You will collaborate with the facility Medical Director, attending physicians and nursing staff to deliver high-quality care. You will communicate with and counsel family members on treatment options when there are material changes in status.

• Provide primary, urgent and preventive care for residents in assigned skilled nursing facility (and may assist with assisted living facility residents).
• Verify admission orders and conduct initial assessments; perform physical exams and medication reviews.
• Collaborate with Medical Director, attending physicians and nursing staff to deliver high-quality care.
• Counsel family members on treatment options during material changes in patient status.
• Work as part of the medical staff to manage medical needs of residents and be the primary point of contact for emergencies and urgent care visits.

• Valid Nurse Practitioner License in Alabama
• DEA License
• Nurse Practitioner national certification
• Clinical background in adult, family or geriatric specialties
• Vaccination against COVID-19 is required for all new hires unless the individual qualifies for remote, religious, or medical exemption

• Experience in internal medicine, gerontology, or cardiology
• Excellent administrative, organizational and verbal skills
• Ability to work independently
• Detail oriented; dependable and reliable
• Nursing experience in LTC / SNF is a plus
