Job SummaryA qualified Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. Services are provided in accordance with physician orders and a plan of care under the direction and supervision of a Registered Nurse, following Agency policies.
Qualifications- Graduate of a state‑approved school of practical (vocational) nursing and currently licensed in the state(s) where practicing.
- Minimum of one (1) year of nursing experience (preferred).
- Acceptance of the agency’s philosophy and goals.
- Ability to exercise initiative and independent judgment.
- Understands and adheres to established policies and procedures.
- Implements nursing care plans for each patient.
- Provides nursing services, treatments, diagnostic and preventive procedures as assigned.
- Initiates preventive and rehabilitative nursing procedures as appropriate.
- Observes signs and symptoms, reports to physician and RN about treatment reactions, including drug and changes in patient’s physical or emotional condition.
- Teaches and counsels the patient and family/significant others regarding nursing care needs and related problems at home.
- Evaluates with a registered nurse the effectiveness of the LPN’s nursing service to the patient and family under RN guidance.
- Maintains accurate and complete records of observations, treatments and care.
- Participates in medical record audit as assigned.
- Attends staff meetings, patient care conferences and in‑services as scheduled.
- Takes on‑call duty, including nights, weekends and holidays as assigned.
- Submits any changes in schedule to the Director of Clinical Services/Nursing Supervisor daily.
- Participates in patient care conferences to discuss the need for involvement of other health team members (e.g., physical therapist, speech/language pathologist).
- Prepares clinical and progress notes.
- Assists the physician and RN in performing specialized procedures.
- Prepares equipment and materials for treatments.
- Observes aseptic technique as required.
- Assists the patient in learning appropriate self‑care techniques.
Works indoors in the agency office and at patient homes, traveling to and from patient homes as needed.

Director of Clinical Services, Nursing Supervisor, and Registered Nurses.

Lifting RequirementsAbility to perform the following tasks if necessary: participate in physical activity; work for extended periods while standing and engaging in physical activity; perform heavy lifting; do extensive bending, lifting, and standing on a regular basis.
