This position reports to the Manager of Care Management. It is responsible for the day‑to‑day operations of Care Management including daily staffing scheduling, supervising and mentoring staff, ensuring standardization of care management, responding to customer requests and problems, and providing leadership for compliance and performance improvement.

• Bachelor's Degree or equivalent experience accepted in lieu of degree
• Related certification
• Registered Nurse with current Ohio licensure
• 3–4 years experience in a related field

• Participate as a member of the interdisciplinary team to ensure selected clinical quality criteria are met, including discussions and education on core measure requirements.
• Achieve a target query rate of 12% and provide supervision to social work staff while ensuring all rules and regulations are followed.
• Maintain CDMP worksheets that are concise, professional, and reflect accurate DRG shifts, keeping them updated for monthly reporting.
• Assist in managing team member daily schedules, adjusting them as needed to ensure adequate coverage.
• Conduct interdisciplinary team rounding on units and MDR rounds to address patient needs, collaborating with physicians and ensuring timely patient care.
• Stay available to address any workflow or patient discharge disposition issues and concerns.
