BAYADA Home Health Care is seeking an experienced Physical Therapist, PT for a full-time opportunity providing home health visits from our Williston, VT office. This office serves adult and geriatric clients across Washington County. A minimum of one year of clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:
- Make home visits to clients in designated areas.
- Perform diagnostic tests, including mobility, strength, balance, and safety assessments.
- Develop and implement individualized care plans with therapeutic exercises, gait training, and interventions.
- Reassess and modify care plans; collaborate with nursing staff.
- Educate clients and families on rehabilitative activities to promote health and independence.
- Document observations and interventions using electronic medical records.
Qualifications:
- Vermont Physical Therapist license
- At least one year of recent PT experience
- Graduation from an accredited program (American Physical Therapy Association or equivalent)
- Ability to work independently and manage time effectively
- Strong interpersonal and organizational skills
- Computer proficiency; EMR experience preferred
