The Wound Care Nursing Team Lead will assist in the supervision and coordination of all activities related to the Wound Care Nursing Team. The Team Leader provides clinical coordination of care related to skin integrity and wound care across CVMC and WDR, and to the outside community. The Team Lead delivers professional nursing care to patients from newborn through end‑of‑life, across varying states of health, through assessment, planning, implementation, and evaluation of the patient’s health needs and goals.
The Team Leader also provides direct care to patients and families, while leading teaching and preparation for continuing care. The Team Lead serves as the clinical expert and resource for WDR residents, ED, ACU and CVMC inpatients, medical team staff, and the organization ough mentoring, educational programs, and exemplary clinical and customer service, the Team Leader demonstrates clinical leadership skills.
The primary clinical duties will be performed at Woodridge Skilled Nursing & Rehabilitation, with oversight of the nursing wound care team at both Central Vermont Hospital and Woodridge Skilled Nursing & Rehabilitation.
- Current RN licensure recognized by the State of Vermont.
- BSN required.
- CWCN Certification (or within one year of hire) required.
- Experience in wound care and skin integrity management.
