Position: Full-Time RN Case Manager in Ottawa

In this role, you will be part of a multidisciplinary health care team focused on comprehensive patient care. Your responsibilities include conducting detailed assessments, formulating care plans tailored to individual needs, and ongoing monitoring to ensure effective health management. Key Responsibilities:

• Conduct detailed case management assessments

• Develop individualized and comprehensive care plans

• Monitor client progress and suggest modifications

• Collaborate within a multidisciplinary health care team

• Ensure adherence to nursing standards and practices

Requirements:

• Baccalaureate Degree in Nursing

• Current nursing license from provincial college

• 3 years clinical experience, including case management

• Certification in Basic Life Support (BLS)

• Commitment to annual BLS recertification
