Care Manager

Responsibilities
Providing primary care management, nursing support, coordinating & scheduling care for clients to ensure they maintain dignity, independence, and joyful living.
Developing and maintaining dynamic care plans and schedules for clients.
Leading/coaching teams of caregivers and interacting with multidisciplinary team (physiotherapists, massage therapists, doctors, etc.).
Teaching new graduate PSWs various elements of caring for clients.
Managing own INTEGRAcare clients, establishing and modifying dynamic care plans and schedules.
Managing care teams by planning and scheduling caregivers with a focus on continuity and consistency of care.
Helping teach new hires program and assisting in orientation of new hires.
Being part of on‑call rotation.
Participating in company and team‑building events.
Qualifications
Valid CNO registration in good standing.
1–2 years of nursing experience (preferred).
Bilingual in English and French.
Great attitude and willingness to work in a fast‑paced environment.
Strong attention to detail and desire to "Close the Loop".
Good written and verbal communication skills.
Passionate about providing clients and caregivers with highest quality care and support.
Open to new learning opportunities, coachable and dependable.
Valid driver’s license in good standing.
Please note – this role will involve in‑person paid training as per a mutually agreed schedule at our Toronto office at 396 Moore Avenue.
Type of Position: Full-Time | Annual Salary: $75,000-$77,000 + Benefits |
