As a Case Manager and Care Coordination Team Leader at Alta Med, you will manage PACE participants in an ambulatory setting. This role requires a strong background in geriatric nursing and case management, emphasizing your ability to provide quality care to diverse populations.
Key Responsibilities:
• Manage care for all PACE participants effectively
• Coordinate services within ambulatory care settings
• Facilitate communication between patients and healthcare providers
• Monitor patient outcomes and adjust care plans accordingly
• Lead a team focused on quality geriatric care
Requirements:
• Current California RN License required
• 1 year of Geriatric Nursing experience preferred
• Experience in managed care or case management preferred
• Valid BLS certification per AHA or Red Cross guidelines
