Key Responsibilities:
• Deliver palliative care focused on patient quality of life
• Collaborate with various health care team members
• Support families through illness and bereavement phases
• Promote effective problem-solving within the team
• Maintain excellent patient documentation and communication
Requirements:
• Certified with the College of Nurses of Ontario
• Minimum 1 year in palliative care settings
• Exceptional communication and organizational skills
• Strong ability to work within an integrated team
• Current CPR certification required
