Key Responsibilities:
• Perform accurate client assessments and design effective care plans
• Serve as the lead contact for client care in collaboration with other professionals
• Guide clients through care transitions and connect them to local resources
Requirements:
• Valid registration with the College of Nurses of Ontario
• A current CPR certificate and valid driver’s license
• Commitment to evidence-based practice and community service
• Criminal background check and updated immunizations required
• Flexibility to work various shifts, including weekends
