In this role, you will assess clients thoroughly and work closely with the healthcare team to implement and modify care plans based on individual needs. Key Responsibilities:
• Gather information through structured interviews with clients
• Assess client environments to inform care strategies
• Update and document care plans collaboratively with the team
• Provide health teaching based on adult learning principles
• Incorporate research findings into daily nursing practice
Requirements:
• Registered in good standing with the College of Nurses in Ontario
• Excellent technical skills including IV procedures
• Reliable vehicle and valid driver’s license required
• Effective problem-solving and documentation skills
• Wound care experience is preferred
