Join Southbridge Ottawa as a full-time RPN Care Coordinator, focusing on behavioral support for residents with complex needs. This role is dedicated to enhancing the quality of life in a compassionate environment.
In this vital role, your responsibilities will involve delivering person-centered care, conducting assessments, and coordinating with a team to ensure safety and well-being. You will actively participate in the development of care strategies aimed at mitigating risks while providing quality support to residents with challenging behaviors. Decision-making and communication skills will be key in achieving successful outcomes.
Key Responsibilities:
• Promote self-esteem and abilities of residents
• Perform assessments to identify behavioral risks
• Communicate care strategies with external managers
• Coach and train staff in responsive behavior management
• Attend quality improvement meetings
Requirements:
• Registered Practical Nurse with current competency certificate
• At least two years' experience in a similar role
• A background in human psychology is a plus
• Bilingual in English and French is advantageous
• Strong collaborative and problem-solving skills
