This full-time position within Personal Support Services requires an RPN to coordinate and promote exemplary client-centered care. You will conduct comprehensive assessments, provide mentorship to staff, and develop care plans in collaboration with clients and interdisciplinary teams.
Your role emphasizes clinical judgment, customer service, and commitment to regulatory compliance.
Key Responsibilities:
• Conduct client assessments and reassessments regularly
• Develop and evaluate individualized, client-centered care plans
• Supervise and support Personal Support Workers delivering care
• Monitor service delivery through visits and reviews
• Collaborate with healthcare professionals for optimal outcomes
Requirements:
• Graduation from an accredited nursing program
• Current registration with the College of Nurses of Ontario
• Minimum 12 months' experience in home or community health
• Strong interpersonal and conflict resolution skills
• Valid driver’s license and CPR certification
