Position: Family Resource Worker, Social Work Team, Ambulatory Care - Permanent Full Time (1.0)

POSITION SUMMARY
In collaboration with the social work team, the family resource worker is responsible for supporting families in need of additional resources while providing care for their child s includes coordinating hospital and community services for and with families; receiving and processing all requests for financial assistance from the CHEO Foundation; advocating on behalf of families for needed services, and assisting with managing the social work funds.

RESPONSIBILITIES

Work closely with CHEO Social Workers to coordinate hospital and community resources for families

Assist Social Workers and families with the completion of applications for services and funds (compiling forms, assisting families in completing forms, mailing applications to appropriate sources)

Prepare appropriate referrals and applications including written documentation to community resources (non‑insured health benefits for First Nations and Inuit, assistance for children with severe disabilities, Easter Seals, housing, Children’s Wish Foundation, Christmas Exchange, people’s services, food banks, para‑transpo, snowsuit fund, Rotary Home, respite plus, Salvation Army, and other clothing sources)

Organize resource packages, manage resource information on the shared drive, ensure adequate supply of current application forms and other documentation (ordering and photocopying) as required

Provide follow‑up as needed to ensure that applications for services are being processed in a timely fashion, especially if parents are concerned

Assist families during inpatient hospital stays at CHEO with practical issues related to the hospitalization: parking, transportation, accommodation and meals

Pursue special funding requests and assist with identifying resources to facilitate discharge (such as transportation, medication, emergency household needs, equipment)

Assist with special projects and other related duties appropriate to the position as assigned

Participate in staff development activities and attend program and social work meetings (when appropriate)

Update patient records to ensure proper documentation of services received

Because of the changing nature of the job, duties and responsibilities may be added or deleted from time to time

QUALIFICATIONS

Vulnerable Sector Check (Essential)

College diploma in Social Services or equivalent (Essential)

Three years of experience working directly with families whose children have a medical condition, special needs or disability (Essential)

Solid knowledge of community resources and knowledge of financial services designed to assist children with illness, special needs and disabilities (Essential)

Strong interpersonal and communication skills (Essential)

Excellent problem‑solving and organizational skills (Essential)

Ability to work as part of a team (Essential)

Computer literate and good writing skills (Essential)

Police record check for the vulnerable sector (Essential)

Direct experience working with families who are under stress (Essential)

Experience with clients from diverse backgrounds (Preferred)

Bilingualism (French and English) (Essential)
