Key Responsibilities:

• Collaborate with social workers to coordinate community and hospital resources

• Help families complete applications for financial assistance

• Document referrals and applications to access community support

• Maintain and organize resource information and documentation

• Address practical concerns for families during hospital stays

Requirements:

• Vulnerable Sector Check (essential)

• Three years of experience with families facing medical challenges

• Knowledge of community financial services

• Excellent organizational and communication abilities

• Bilingual in French and English
