Based in Little Burgundy and serving participants across Greater Montreal, DESTA offers access to a hub of services and partnerships that empower the Black community through programs rooted in social and economic justice. The Opportunity

The Reentry Case Manager will provide service navigation, referrals, connections, and supports for participants in legal support, housing, employment, health, mental health, and other social services. This role advances community reintegration for incarcerated and formerly incarcerated individuals.

Responsibilities
  • Conduct intake, assessment, case management, consultation, support, and referrals for incarcerated and/or formerly incarcerated clients.
  • Deliver intensive, community-based, client-centered, and strengths-based case management to promote successful reintegration.
  • Establish and maintain partnerships with organizations in Little Burgundy/Sud-Ouest and Greater Montreal; develop referral networks and tracking systems.
  • Identify and coordinate resources and service planning collaboratively with community partners/agencies.
  • Ensure service delivery aligns with agency policies, procedures, and accreditation standards.
  • Collaborate with the Reentry Services Team (Coordinator, Employment Counselor, Social Worker, Program Administrator) to support integrated client services based on social determinants of health.
  • Participate in team and staff meetings regularly.
  • Engage in special events, community activities, and DESTA committees as appropriate.
  • Contribute to yearly service planning and program development meetings.
  • Perform other duties as assigned.
Qualifications
  • Post-secondary education in psychology, sociology, social services, or related discipline.
  • Relevant case management experience.
  • Experience working with incarcerated/formerly incarcerated individuals or in correctional environments.
  • Knowledge of Canada's criminal justice system, correctional services, restorative justice, and supports for this population.
  • Skilled in case management, counseling, crisis intervention, and group facilitation.
  • Ability to work independently and collaboratively within a team.
  • Strong organizational and time management skills.
  • Excellent written, verbal, interpersonal, and human relations communication skills.
  • Highly developed conflict resolution and problem-solving abilities.
  • Commitment to anti-oppressive, trauma-informed frameworks.
  • Sensitivity to mental health/addiction issues and experience with victims of violence and abuse.
  • Knowledge of community resources, social programs, and relevant legislation.
  • Proficient in English;
    French bilingualism is an asset.
Additional Assets
  • Experience in a non-profit environment.
  • Understanding of challenges facing the Anglophone Black Community in Montreal and issues of racial equity and social justice.
  • Bilingual (English/French).
