OverviewVOAC recognizes the unique needs of older adults who require a wheelchair ramp to safely remain at home, and also supports preschoolers whose families need additional resources. VOAC assists veterans in exiting homelessness and provides a safe haven for survivors of domestic violence. Wherever VOAC goes, it combines relationship‑building, volunteers, and quality services to lift and support neighbors toward self‑sufficiency.
ResponsibilitiesThe Service Coordinator is directly responsible for the service provision and housing assistance for residents in the Affordable Housing Portfolio. The position serves seniors aged 62 and older at one of the affordable housing sites in Montrose. Service modalities include Trauma‑Informed Care, Harm Reduction, and Motivational Interviewing.
- Ensures residents’ voices are heard and incorporated into programming.
- Facilitates referrals on residents from the Property Manager, family members, service providers and residents themselves.
- Assesses services needed by residents through surveys, visitations and group meetings, and completes an assessment to identify unique needs.
- Provides residents with information on specific services or program providers, offering several options whenever possible.
- Encourages residents and/ or family members to arrange necessary services to maintain independence.
- Coordinates and monitors providers’ services to assure quality of care and cost effectiveness; advocates for residents per their wishes.
- Maintains an up‑to‑date resource guide on service providers, familiarizing with local resources such as the Area Agency on Aging.
- Maintains and submits accurate records on all residents and services received.
- Develops support groups and workshops in response to needs and facilitates or co‑facilitates these groups regularly.
- Assists in relocating residents to a higher level of care as needed.
- Supports team members by serving as a resource for problem analysis and staying abreast of current issues and theories.
- Implements the Service Coordinator program in accordance with HUD and VOA guidelines.
- Attends orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops as requested.
- Documents all client interactions and required eligibility and demographic information.
- Maintains confidentiality and participates effectively within and across teams.
- Completes annual CEU requirements and follows the Social Work Code of Ethics.
Minimum Qualifications
- Bachelor’s degree in human services, social work or a closely related field, or related experience.
- At least one full year of working with senior citizens and/or people with disabilities.
- Willingness to obtain and maintain first aid, AED and CPR certification.
- Criminal and civil background check within 30 days of hire.
- Excellent interpersonal and organizational skills.
- Proficiency using Windows and database applications.
- Mental health experience.
- Bilingual in a language represented in the resident community;
Spanish is a plus.
$21.00–$24.00 per hour (based on experience)
Working Conditions andPhysical Requirements
- Indoor office environment with prolonged periods of sitting.
- Local travel for training and meetings; out‑of‑state travel once per year for conference.
- Physical demands include bending, stooping, kneeling, reaching, squatting, lifting 30–50 pounds, balancing, standing, walking, and visual and hearing requirements.
Part‑time, 20 hours per week. Monday to Friday. Hours are flexible.
