Role Overview

Senior Community Care of Colorado (SCCCO) is recruiting an Enrollment Specialist to support our PACE program in Montrose, CO.

• Obtain initial admission information, coordinate intake processes, and provide referrals to community agencies.
• Promote the PACE program to potential participants and partner agencies.
• Assess participants in various settings, maintain written records, and schedule enrollment meetings.
• Participate in interdisciplinary team meetings and provide clarifying information during assessments.
• Help participants understand enrollment agreements and financial eligibility.
• Monitor Medicaid eligibility, re‑certification timelines, and facilitate applications.
• Collect data and prepare CMS‑required reports.
• Identify and address process problems to improve intake and assessment.
• Maintain relationships with key referral sources and community partners.
• Support quality improvement initiatives, infection control, and privacy protocols.
• Assist in marketing activities, presentations, and fundraising efforts.
• Perform related duties as needed.

• Bachelor’s degree in humanities, health, or human services, with experience in sales, customer service, or social work.
• At least two years of marketing or admissions experience in a healthcare setting, or in a senior‑care referral role.
• Minimum one year working with frail or elderly populations.
• Medical clearance for communicable diseases and up‑to‑date immunizations.
• Valid driver’s license and reliable transportation within 100 miles.
• Clear background check.

• Effective presentation of PACE philosophy and program details.
• Knowledge of elderly needs and community resources in Montrose and Delta counties.
• Strong written and verbal communication.
• Understanding of Medicaid, Medicare, and managed care systems.
• Ability to assess financial status and enrollment eligibility.
• Proficiency with computers and electronic documentation.
• Ability to work under pressure, meet deadlines, and maintain detailed records.
• Commitment to privacy, infection control, and quality improvement.
